Court Rental Request Form

COMPLETING THIS FORM DOES NOT CONFIRM A BOOKING

The Varsity Court default setup is no bleachers or dividers, 1 court lengthwise. Rentals begin on the hour and end 5 minutes before the hour. All setup and tear down must take place during the rental period. Basketballs are not provided by the facility. Please bring your own.

Payment in full and a signed rental contract are required before the booking can be confirmed.

If any aspect of this form is not completed truthfully, the event may be canceled.
